Your Privacy Rights

A straightforward process for asking questions about your personal information and exercising applicable privacy rights.

Chaperone provides a straightforward process for asking questions about your personal information and exercising applicable privacy rights.

Submit a privacy request

Use the secure request form on this page or email privacy@chaperone.health.

Requests You Can Make

Access

Ask whether Chaperone maintains personal information or consumer health data about you and request access to covered information.

Portable copy

Request a portable copy of eligible personal information we maintain about you, where applicable law provides this right and the information is technically feasible to provide in a usable format. You can submit this request through the same privacy request form or email address.

Correction

Ask us to correct inaccurate personal information where applicable.

Deletion

Ask Chaperone to delete personal information and consumer health data associated with you, subject to limited legal, security, fraud-prevention, safety, recordkeeping, or other permitted exceptions. Verified consumer-health deletion requests are processed within 30 days.

Withdraw consent

You may withdraw consent for processing of sensitive or consumer health data where our processing relies on your consent. We will stop the applicable consent-based processing as soon as practicable and no later than 15 days after receiving a valid request. Withdrawing consent may make features that require sensitive-data processing unavailable.

Ask about disclosures

Where applicable, you may request information about certain third parties to which Chaperone has disclosed your personal information or consumer health data.

Appeal a decision

If we deny a privacy request, you may appeal by emailing privacy@chaperone.health with the subject line “Privacy Request Appeal.” Instructions for appealing will also be included in the response to a denied request.

What Happens After You Submit a Request

  1. We acknowledge and review the request.
  2. We may ask for information reasonably necessary to verify your identity or authority to act for another person.
  3. We generally respond to verified requests within 45 days, subject to shorter requirements described above and any legally permitted extension.
  4. If we need a legally permitted extension, we will explain why and provide an updated expected response date.
  5. We will not unlawfully discriminate against you for exercising an applicable privacy right.

Authorized Agents

Where applicable law permits an authorized agent to submit a request for you, we may request documentation showing the agent’s authority and may separately verify your identity.

Information We May Need to Keep

A deletion request does not always require deletion of every record. We may retain limited information when reasonably necessary and legally permitted for security and fraud prevention, maintaining evidence that a request was completed, legal obligations, dispute resolution, protecting Chaperone or others, safety-related recordkeeping, and maintaining necessary system integrity or backups until they rotate out through normal retention.

Information retained under an exception is not used for unrelated purposes. Information that has been de-identified is not maintained as identifiable account data.

Submit a Privacy Request

If you have an account, using the address on the account helps us verify the request faster. We do not confirm whether an address has an account.

Please do not include health details here — we only need enough to identify the request.
